Submission of IACUC Modifications for Administrative Changes

Announcements

Administrative modifications include changes to:

  • Non-key personnel
  • Locations outside of the vivarium where animals are used (lab areas)
  • Funding

Go to RMS Website

Additional Information & Tips

  1. Login to your protocol in RMS
       
  2. Create a Modification in RMS
      
  3. Check-Out the Document (version with doc status "Modification"), then click into the protocol
      
  4. Fill out the Modification Tab in RMS, indicating all proposed changes
    (this tab replaces our old request form!)
     
  5. Email IACUC@uci.edu to let us know you submitted a mod

  • Administrative modifications include changes to:

    • Funding
       
  • Addition of new personnel requires completion of training requirements

    • CITI Training
      • Log into CITI and complete the required training, along with any other protocol-specific modules.

        For guides on how to enroll, contact the IACUC Office at IACUC@uci.edu 
    • LAOHP Questionnaire
      • Enroll or opt out of the Laboratory Animal Occupational Health Program (LAOHP) Questionnaire
        (instructions on how to opt out are within the questionnaire)
      • This questionnaire needs to be updated annually
      • For questions about LAOHP, contact EH&S at (949)824-3757

    It is recommended to have these completed prior to submission!   

  • For more information about modifications, refer to the IACUC Policy on Modifications

Notes about Personnel Additions:

  • Personnel MAY NOT handle live animals until they have been officially added to the protocol
  • Requests for vivaria access may require completion of additional species-specifc CITI training modules - ULAR will let you know which additional ones you need to take
